Short Term Locum Diabetes Nurse

Monkfield Medical Practice (MMP) is a friendly, modern, forward thinking GP surgery providing sustainable healthcare to Cambourne with over 14,000 patients. You will be joining a large cohesive multi-disciplinary team of General Practitioners, Nurse Practitioners, Practice Nurses, Health Care Assistants, Phlebotomist, Management and Administrative staff, Clinical Pharmacist, Social Prescriber, First Contact Physiotherapist and a research team.

We are looking for a short-term locum diabetes nurse for two days a week (up to 15 hours) would expect independent running of diabetes clinics for all types of diabetes to optimise their medication, chronic disease management and cardiovascular health, as well as achieving QOF targets. Role can be expanded to managing QOF on other cardiovascular diseases if of interest. We would aim for the staff member to support diabetes knowledge amongst staff and add to the culture of education.

We are a training practice with four tier 3 trainers, and a teaching practice taking University of Cambridge Medical Students and Anglia Ruskin University Nursing Students. We encourage innovation and improvements, for example, we run a novel community-based palpitations diagnostic pathway with hand-held ECG devices and are upscaling it to the region. We achieved the status of being a Research Practice in July 23 and now deliver several research studies across Meridian PCN with a highly active and experienced team. We support portfolio nurses with extra roles. The partners themselves are skilled in education and research.

We have on site PRISM, ultrasound, midwife, health visitors, physiotherapy, podiatry, SLT, smoking cessation, vasectomy service, dementia support, short term alcohol reduction support.

Position:                    Patient Services – Care Navigator

Location:                    Sackville House, Sackville Way, Cambourne, Cambridge

Days:                          Monday, Wednesday & Friday

Hours:                        27.5 hours Between 0745 – 1800 or 0800 - 1815

Start Date:                  As soon as possible

Salary:                        £12.21 per hour  

 

Monkfield Medical Practice (MMP) has a patient population of 14000 and is led by 3 Partners and their multidisciplinary team who work together within a close team, supporting each other to provide safe, high quality, sustainable healthcare to Cambourne.

We are seeking a part-time Care Navigator to join our friendly team, to answer the telephone, to greet patients face to face and assist patients in accessing the appropriate service or relevant healthcare professional in a courteous, efficient and effective way.

You will provide general administrative assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.

You will play a key role in the continued development of MMP, working closely with the practice team in implementing systems aligned to the practice’s values and purpose, especially during this exciting time of expansion due to the continued growth of Cambourne. 

You will have proven customer service experience and or reception environment and have the ability to multi task within an environment that is unpredictable. You will have a passion for providing excellent customer service when expectations can vary, whilst remaining calm, polite and professional. Excellent IT skills are required. Previous GP and/or NHS experience beneficial.
